This Imprinted Detailed DVIR w Illustrations Roll-on/Roll-off Inspection Report 2-Ply Carbonless Snap-out Format 159-FS-I2 to help comply with 49 CFR §393.134.

Simply check off parts that need repair. Includes a space for the inspector to note and comment on each item that needs repair. Also includes three illustrations of a typical container that the inspector can use to mark the area that needs repair. FMCSA's rules for securing roll-on, roll-off, and hook lift containers are printed on the back of each form as a reference.
